Please fill out and submit the form below in order to request your services to be moved by your current utility provider(s). Note that all fields marked with asterisks(*) are mandatory and therefore, must be completed before you can submit the form.
Also note the followings:
- Your service providers may reach out to you as part of their process for fulfilling customer move requests.
- By submitting your move request through Muvar Inc., you confirm that you have the right to move the service(s) and you authorize Muvar Inc. to contact your provider on your behalf.
- By submitting your request through Muvar Inc., you automatically grant Muvar Inc. the right to contact you during and after your move requests have been completed in accordance to our terms of services and privacy policy.
